A patient companion app for a hospital network of 40 clinics, built offline-first for low-connectivity regions.
The problem
Patients missed appointments and medication schedules; the existing portal was desktop-only and unusable on the 3G connections most patients had.
What we did
Field research across six clinics shaped an offline-first React Native app with local-first sync, SMS fallback for reminders, and a HIPAA-aligned backend.
The result
No-show rates fell by more than a third and the app now handles over 200,000 monthly appointment interactions.
They went to six of our clinics before drawing a single screen. The app works on a 3G connection in a rural waiting room, which is the only test that ever mattered to us.
